CarDon’s Home Transition program

Jill Rusk, RN, Director of Business Development, CarDon & Associates explains CarDon’s goals are set on admission by evaluating the patient’s prior level of  function on admission.

During their recovery, they assess how well patients are able to perform activities of daily living such as~ meal preparation, grocery  shopping, bathing , tub transfers, dressing, car transfers, medication management, ambulation on different surfaces and completing executive tasks such as bill pay. Once this is completed successfully in a setting in the  transition apartment they take the patient and family to their own home and evaluate for safety, lighting and additional  items that need to be implemented prior to discharge.
