Patients back home from long-term care pose challenges
INDIANAPOLIS (WISH) — The transition from a long-term care facility back home can be overwhelming for both former patients and their caregivers.
News 8 spoke with Dr. Jennifer Carnahan, research scientist at the Regenstrief Institute, about the challenges former patients face upon discharge, why the first few days back home are critical and what needs to be done to set them up for a successful transition especially during the COVID-19 pandemic.
